SHAPE Idaho TOY Award Nomination Form

If you have been nominated for a SHAPE Idaho TOY Award, click on link below to submit your application. SHAPE Idaho requires you to submit this application by May 9th, 2025. Please be aware that you will be redirected to the SHAPE America TOY Application in which you will specify that you are applying for your respective STATE TOY Award. You will be asked to provide personal information, education/career information, professional affiliations/activities, and upload two (2) letters of support documents. Winners of their respective award category will be notified by May 19th, 2025:
